FAMILIES & PROFESSIONALS

The right people.
A shared understanding.

Thoughtful collaboration can create continuity around a young person, while respecting their voice, privacy, and individual circumstances.

COLLABORATION

Working with the people who matter.

01

Start with youth voice

Involve the young person in conversations and decisions in ways that suit their communication, understanding, and circumstances.

02

Understand each role

Clarify who is authorized to make decisions, receive information, and participate in planning. Keep responsibilities clear.

03

Respect privacy & consent

Share relevant information through appropriate channels and according to consent, authority, and the young person’s care arrangements.

04

Plan for continuity

Discuss routines, relationships, priorities, and upcoming changes. Thoughtful communication helps reduce avoidable uncertainty.

A USEFUL STARTING POINT

Questions worth asking.

Is the program a good fit?

Ask about the confirmed program’s scope, eligibility, location, available supports, and limits. A service should be considered in relation to the individual’s needs rather than its name alone.

How will the young person be involved?

Discuss how preferences, goals, communication needs, and concerns will be heard and reflected in planning.

What does communication look like?

Ask who the appropriate contact will be, how updates and concerns are handled, and what information can be shared with each person.

How are transitions prepared?

Explore the steps for introductions, planning, continuity of relationships, and relevant information sharing.

How can concerns be raised?

Ask for the confirmed program’s concerns process, contact roles, and options for follow-up.

Referral pathways and program-specific information will be published when confirmed. Everrise is not currently advertising immediate placement availability.
